# 6. Biểu mẫu

**1:** Ấn nút + để như hình trên&#x20;

**2:** Chọn Biểu mẫu (Lưu ý: có thể tùy biến và chọn bao nhiêu Biểu mẫu tùy theo mục đích khác nhau)

**1: Câu hỏi** - Khung câu hỏi đang có có sẵn

**2: Loại câu hỏi mới** - Thêm các loại câu hỏi theo định dạng khác nhau

**3:** Chế độ xem trước sau thiết kế xong sẽ ra sao

**4: Xem phản hồi** (kèm theo điểm đánh giá nếu làm bài kiểm tra) của khách hàng/đối tác... sau khi điền biểu mẫu (form).

**5: Bật chế độ kiểm tra** - sẽ đưa ra bài test để đánh giá năng lực của dự án, hiểu biết và đạt tiêu chuẩn cho một sự kiện gì đó.

**1: Làm mới** - Sẽ thay mới làm toàn bộ link, QR code (Link cũ và QR Code cũ sẽ không hoạt động được nữa)

**2: Dừng chia sẻ** - Sẽ không chia sẻ nữa, link/url cũng không hoạt động

**3: Link** - Chia sẻ theo link - chỉ việc copy ở vị tría 6 trên ảnh

**5: Nhúng**

Cho phép nhúng vào bất cứ website (wordpres..v.v.) để khách hàng, đối tác điền thông tin

**Quan trọng:** Sau khi khách hàng/đối tác điền thông tin, thông tin sẽ được lưu trữ vào trang Danh sách (Trang nguồn).
